We authorise Emergico Migration to apply for the Australian Federal Police (AFP) check on my/our behalf for the purpose of my/our application.

This application will be lodged under Code 33 and will be a ‘Complete Disclosure’ application.

We understand this is required for all persons included in the application who are 16 years or older and have been living in Australia for at least 12 months in the last 10 years.

We understand that identification documents will be uploaded to the AFP Online Application facility, along with all names known by and address history.

We understand that it is an offence to provide false or misleading information in this application or omit to provide information that may result in this application being false or misleading.

We understand the AFP Clearance will be sent to Emergico, and the originals will be provided to us at the conclusion of the visa application process (unless requested earlier).

We understand that the AFP clearance must list all names that I have ever been known by – which may include:

- Formal names (e.g. previous married or maiden names);
- Variation spellings of current names;
- Informally adopted names or nicknames (such as an English version of your name)
